Also failing to note.. men NPCs can be, and are, killed.
And that there are games where people aren't killed. For instance, the many variations of the SIM game universe. Such as SimCity.

The whole joy of an open world game is to do anything, specifically things you would never do in real life - like kill with reckless abandon. Nothing's going to pull you out of that gameplay faster than some invisible fence guarding "socially unacceptable behavior".


There's nothing stopping McIntosh from creating his own world with doctors (and lawyers) doing their thing.


Actually... there is a whole large GTA world of RP with medics, cops, bankers, etc. and rules about indiscriminate killing and consequences.

Imagine 30 levels of HALO and your only objective in each level is to run around and "heal" all the NPC's shooting at each other.
