I'm not so sure that the "in Mexico" part has a place in the causation matrix. I think the root cause analysis will pinpoint "in water" and "under a guy with a bad ticker" as more highly placed risk factors.

I for one avoid At least half of those nearly all the time. It's just a safety thing.
