The wife can't back up without turning the wheel extreme, one way or the other. Park van, with the wheels cocked this way, and the power steering will make some noise. Pull it forward a bit, or just release the steering wheel, quiets down. Hope this helps.

Nope, just when it's cold. And, it's not using any fluid, fluid level is fine.

Driveway is straight in/straight out. Wheels are always pointed straight ahead when car is turned off or started.
