I'd say good chance its real, I saw something like this at a firing range quite a few years back. Guy was shooting a .44 at a 7yd target and hit the metal clip that holds it. Bullet fragmented and a piece came back and buried itself in his arm just under the skin.

I want to know why the hell he was standing down range in the first place! I to have had a couple instances with this sort of thing, had the pellet from a 12G shotgun shell go through the top of my arm a couple years ago, and had a .22 come flying back at me and my friend(Sir.Golem), we were standing about 2 feet from each other, side by side, and it went between our heads.

You know it's things like this that really confirm the frequency of Darwinism at work while at the range. I think I have seen 2 or 3 of these kinds of videos. We can only hope the followup shot comes before he has a chance to reproduce.
