I have stepped on sharp objects, stubbed my toe, even once dropped a tow motor propane tank and crushed my big toe. They all hurt! I could make enough of an educated guess to deduce that a gunshot to the foot would be quite painful.

If he is stupid enough to do something like this deliberately, he is probably stupid enough to believe that his brain is in his foot (as opposed to having been up his rear and been, well, ejected).
