    I have all my forms filled out. I have been finger printed and the IMPD has signed my form 4. On the finger print card in the top left area I signed my name, below that i put my address. Below that is a spot for the date when the prints were taken and the signature of the official that took the prints. That part is all gtg but below that is a spot that says employer and address. Is that my employer and address or the employer and address of the official that took the prints?

    It is the employer of the person being printed. But it doesn't matter for NFA purposes.

    That is a space for employer info if you are being printed for a job background check.

    Well its blank right now. Can I leave it blank? Will that slow down my process? Thanks for all the replies.
