I previously had a handgun license in Indiana - back in the early 80's. I don't remember what the exact year was or what the number was. What information should I put on the form?

I did the same thing. Obviously my Indiana permit automaticly expired when I moved to Kentucky. I obtained my Kentucky Permit. When I moved back to Indiana and applied for my Indiana LTCH, I put the approximate dates for my Indiana Permit. I don't know if it really mattered however, my application still took a long time.

I have a similar question. My future father in law applied a week before i did and i think i got mine tonight (will see when i got home, just got a call saying i got something from the ISP) He had a permit back in the 80s couldnt remember when let alone the number so he just put no, will this cause it to get rejected and he will have to make the change?
