Well, I've never heard the term "surgery" conflated with "transvestite", but if that's all that this thing wants, who cares? I mean all the State would need would be some needles, thread and a couple of nice patterns. Maybe a sewing machine. Sure, you'd need to use scissors, but I wouldn't call it  surgery per se.
